Project Description:
The Colorado Long-Term Care Ombudsman Program continues to support the work of culture change/person-centered care. Our efforts included:
· Ombudsman attendance at the Colorado Culture Change Coalition, which meets monthly.
· Ombudsman participation in training sessions with our QIO and person centered pilot facilities in the 7th scope of work.
· The 2005 Colorado Long-Term Care Training Conference included a presentation by Pueblo Extended Care Facility on their culture change journey. The administrator, several staff and four residents attended and shared their experience. The session got the highest marks for the conference. You can find more information on Pueblo Extended Care in NCCNHR’s 2004 Residents Rights’ Week publication.
Project Impact:
It creates an opportunity for ombudsmen to get the word out on culture change. Ombudsmen share the experience of Pueblo Extended Care and other culture change homes with consumers and facilities in their regions.
