Project Description:
The Chenango LTCOP saw a need to help the Families be an advocate for their” Loved One’” who are in a Long Tern Care Facility (LTCF) in their County. The Coordinator identified a certified Ombudsman Volunteer who had the skills to work with the creation of coalitions. The two of them worked on the concept of having one Ombudsman assigned to help the families create their own Family Council in the LTCF.
After one year, the Coordinator has seen the need to have two Certified Ombudsman Volunteers assigned the work of helping to create a family council in a LTCF’s in their County.

Project Impact:
- Improved quality of care for the residents in the LTCF
- Resident directed care
- The implementation of “new “ ways of caring for LTC residents due to the ability of the family to advocate and create change
- The education of family members and the Community about the LTC issues and creative solutions
